Com a qualsevol trastorn psicològic, el primer pas, es elaborar un diagnòstic complet i tot i que existeixen instruments psicològics estandarditzats per a aquesta finalitat, el diagnòstic d´ autisme, requereix fonamentalment, observació conductual sistematitzada, cosa que es tradueix en la necessitat de dur a terme una observació detallada i el més natural possible dels aspectes afectiu-relacionals, comunicatius, lúdics i d´aprenentatge.
En termes generals, no es gens fàcil realitzar una avaluació conductual d´ un nen amb autisme, les pròpies dificultats de relació i comunicació, característiques del trastorn, constitueixen un obstacle important per a una observació sistemàtica. Per aquest motiu, es important tenir en compte els següents punts:
- Temps: Hem de tenir present, que aquesta no es pot realitzar en unes poques hores, necessitarem més d´ un dia.
- Experiència: La particular idiosincràsia d´ aquests nens, dificulta una exploració exhaustiva, per això es recomanable que els professionals, estiguin acostumats a tractar amb aquests nens.
- Contextos: Passa habitualment, que els nens amb autisme, manifesten conductes molt diferents en funció del context en el que es troben.
Instruments Diagnòstics:
Tot i que el diagnòstic d´ autisme ha de realitzar-se mitjançant un procés d´ observació, també es necessari fer servir instruments diagnòstics, ja que el seu caràcter estructurat pot ajudar a establir diagnòstics diferencials en casos complexes, així com aportar informació complementària molt útil.
Entre els instrument diagnòstics destaquen el CARS (Schopler y col.,1980) i el ADI-R (Le Couter y col.,1989; Lord i col., 1994) i el ADOS.
El CARS, es una escala conductual de 15 elements que permet obtenir un diagnòstic categorial d´ autisme, mitjançant la classificació del subjecte segons les següents categories: no autista, moderadament autista i severament autista.
Els diferents ítems, fan referència als símptomes fonamentals de l´ autisme i les opcions de resposta son de tipus escalar (1-4), de tal forma, que la puntuació 1 indica que el subjecte manifesta una conducta (en relació amb aquest element en concret) dins de la normalitat, en canvi, si la puntuació fos 4, significa que la conducta expressada es profundament patològica.
El ADI-R, es una entrevista molt extensa de caràcter semiestructurat que proporciona, a part d´un diagnòstic diferencial de l´ autisme, una gran quantitat de dades en relació amb la conducta actual i passada del subjecte. Degut a que es un instrument extens, l´ aplicació del qual requereix molt temps, es utilitzat sobre tot, en el camp de la investigació.
La escala ADOS, es una avaluació estandarditzada i semi-estructurada que mesura la comunicació, la interacció social, el joc i l´ ús imaginatiu de materials per a subjectes susceptibles de patir trastorns de l´ espectre autista.
Consta d´ un conjunt d´ activitats, que proporcionen contextos on l´avaluador pot observar o no la presència de certs comportaments socials i de la comunicació rellevants per a un diagnòstic positiu.
Tractament Psicològic:
Segons els estudis dels que es disposen, els programes terapèutics més efectius son aquells que combinen l´aplicació de tècniques de psicologia conductual i cognitiva. Aquests programes, s´ han d´ adaptar a cada cas particular, però es pot dir que segueixen aquests principis generals:
- Plantejar objectius funcionals.
- Dissenyar un número raonable d´ objectius.
- Realitzar avaluacions periòdiques i objectives dels subjectes.
- Educar en contextos, el més natural i variats possibles.
- Estructurar al màxim els processos d´ aprenentatge.
- Treballar conjuntament amb la família.
El perfil cognitiu específic i diferent a qualsevol altre trastorn implica conèixer les alteracions en:
- Les habilitats socials bàsiques i dificultats pel que fa a les habilitats de la cognició social (com la teoria de la ment).
- La comunicació (per desenvolupar codis simbòlics i per compartir el món intern).
- El processament de la informació (dificultats per donar sentit i coherència a l´ entorn, predir el que passa i generar conceptes més enllà de la percepció en els detalls).
- Les dificultats per a organitzar les seves activitats i la seva vida (amb una necessitat de rutines i dificultats per adaptar-se de forma flexible als canvis, limitacions per planificar-se i una limitada creativitat).
- La percepció sensorial.
A més, les persones amb TEA tenen un perfil cognitiu caracteritzat pels següents punts forts:
- Habilitats visuals.
- Memòria associativa.
- Habilitats musicals.
- Aprenentatges rutinaris.
Les dificultats que presenten les persones amb TEA, no suposen només un retard pel que fa al seu desenvolupament si no que impliquen una percepció diferent de la realitat i de l´ entorn, que afecta a la seva forma d´ adaptar-se i relacionar-se amb aquest, desenvolupant en ocasions conductes poc ajustades i alternatives.
